Our compassionate and highly trained nursing team understand that everybody is unique and so are their care requirements. We use evidence-based practices to provide tailored, comprehensive and flexible services to ensure the safety and wellbeing of our clients.

Our services

Our wide range of community nursing services that can be delivered on an ongoing basis includes:

Medication management

Administration and supervision of oral medication, subcutaneous and intramuscular medication, Schedule 4 and Schedule 8 drugs and other prescribed medications.

Diabetes management

Management and care of type 1 and type 2 diabetes, including insulin and medication administration and monitoring, hypoglycaemia management and patient-centred education regarding self-management of diabetes.

Care management and reviews

Reviews and updates of ongoing care and care plans, and referrals for participants as per their care requirement. 

PEG feeding

Assistance with meeting nutrition, hydration and medication needs via PEG tube. 

Catheter management

Urethral and suprapubic catheter care and maintenance.

Bowel care

Including enema administration and stoma care. Acute and chronic wound care, including wound assessment and management.

Nursing assessments and reviews

Holistic health assessments of NDIS participants to identify their health and care needs to ensure that appropriate and person-centred care is provided. These assessments can also be used as supporting evidence during a plan review. 

Wound care

Acute and chronic wound care, including wound assessment and management. 

General nursing care

Including pain assessment and management, liaising with GP’s and general observations.

Meet Rajani

Registered Nurse – NDIS

I have worked as a registered nurse since 2006, initially working in theatre, followed by aged and disability care. While studying, I also worked in the community looking after a client with disability, supporting her to live in her own home with her mum. This sparked my interest in the disability sector.

Giving you choice and control

My participants are at the heart of what I do. I take time to understand peoples’ needs and use a person-centred approach in my work. I empower my clients to achieve their goals by understanding what they want to achieve and assisting them to succeed. My priority is to ensure that they are safe and their choice and control is respected.
